N2 - Spontaneous herniation of the temporomandibular joint (TMJ) into the external auditory canal (EAC) is a rare occurrence. Typically, TMJ herniations are small and involve a focal defect in the EAC. In this report, we present an unusual case of extensive TMJ herniation measuring 17 mm, originating from a bony defect in the EAC. The patient underwent a successful treatment, which involved the surgical excision of the herniated mass and the subsequent reconstruction of the EAC using conchal cartilage. In this article, we will discuss the surgical techniques employed for repairing TMJ herniation in the EAC.
